All registration options include the registration packet and conference program distributed at the conference as well as conference proceedings.

This option includes admission to all conference sessions; a Tuesday evening reception with hors d'oeuvre and cash bar; and daily continental breakfast, coffee breaks, and lunch (Monday and Tuesday only).

One Day Admission

This option includes admission to all conference sessions, meals, breaks, and special events (such as the reception) offered on the day of your choice.

  1. Complete the ICCBSS Registration Form.
  2. Obtain an appropriate form of payment (see "Terms and Conditions" below.)

    We cannot process registrations by phone. Send registrations once by either FAX or mail-do not use both methods.

TERMS AND CONDITIONS
Payment
Payment in full must accompany registration. Method of payment may not be changed after submission of registration. Acceptable methods of payment include the following:

    Credit Cards
  • MasterCard, Visa, American Express, or Diners Club
  • credit cards are processed upon receipt
    Company or Personal Checks
  • made payable to SEI/CMU
  • must indicate registrant's name
  • will not be accepted via fax
    Completed Purchase Orders
  • accepted only until on-site registration begins on 23 January, 2002
  • must indicate "Advanced Payment Required"
  • must be signed by the designated fiscal officer in your organization
  • for government purchase orders, we recommend using DD Form 1556 as a pre-paid training form, with "Advance Payment Required" in block #37 and fiscal officer's signature in block #29

Discount rates will be honored until 7 January, 2002 (12 midnight U.S. Eastern Time).

Refund, Cancellation, and Substitution
Registration fees are not transferable to other SEI events. To send a substitute or request a refund, registrants must send written notification to the address indicated in the instructions by 23 January, 2002. The SEI will issue refunds (less a $50 administrative fee) for cancellations received in writing by 23 January, 2002. No refunds will be issued after 23 January, 2002. Refunds will not be given for sessions missed due to acts of God.

Registrants who do not cancel in writing by 23 January, 2002 are responsible for payment whether they attend or not. If they do not attend, conference materials will be mailed to them after the conference.

Confirmation and Receipt
The SEI will send confirmation letters within three working days for all registration forms received by 23 January, 2002. Attendees will receive receipts at on-site registration.
